Voluntary Attention Regulates Acute Immune Responses in Humans

Three pre-registered experiments, 57 participants across two independent cohorts. Researchers induced acute skin inflammation using a standardized histamine skin prick test while participants either directed attention inward toward their bodily sensations or outward toward distractors. Inflammatory responses were measured in real time across 20 minutes.

Results: directing attention toward bodily sensations produced approximately 1.5-fold smaller inflammatory responses than distraction — in 90% of participants. The effect appeared within 3 minutes and grew over time. Participants who directed attention inward also showed significantly faster return to baseline.

Two complementary mechanisms were identified. First, a sensory-dependent route: paying attention to the sensation amplified interoceptive feedback, helping the nervous system regulate the inflammatory response more precisely. Second, a top-down parasympathetic vagal route: internal attention significantly increased heart rate variability, indicating greater parasympathetic engagement and activation of the body's anti-inflammatory reflex.

The authors note that turning attention away from pain and discomfort is a common clinical practice — used in hospitals, rehabilitation settings, and pain management. Their findings suggest this strategy, while reducing perceived discomfort, may come at a physiological cost: impairing the body's natural ability to regulate inflammation.

This study provides the first direct experimental evidence that this approach may be doing something physiologically meaningful, not just psychologically.

What stops me is the implication for distraction as a clinical strategy. When we hand someone a screen to watch during a procedure, or encourage them to "think about something else" during chronic pain, we may be trading short-term comfort for impaired inflammatory regulation. The body's natural regulatory loop appears to depend on attention — on the brain actually receiving and processing the sensory signals coming from the site of inflammation.

What I find myself wondering — and I want to be honest that this goes beyond what this study proves — is what this means for people living with chronic pain and autoimmune conditions. Were some of them, over years or decades, dissociated from their body's sensations? Not because they were indifferent to their pain, but because the pain was so overwhelming that disconnection became the only way to function? And if the body's ability to regulate inflammation depends in part on receiving attentional input — if distraction impairs that regulation — then what might a daily practice of gentle, curious, non-judgmental attention to bodily sensation make possible over time?

I don't know. The study doesn't answer that question — it was conducted in healthy people experiencing acute inflammation, not in chronic pain or autoimmune populations. But the mechanistic pathway it identifies is real. And I think the question is worth asking.

The intervention used a Functional Integration® technique in which a practitioner applied gentle pressure to the foot using a hard surface — the "artificial floor" — while the participant lay supine in the scanner.

Researchers examined interoception across four dimensions in relation to self-harm and suicide risk, with heterogeneous results overall. The clearest signal: trust in bodily signals and the ability to regulate distress through attention to internal sensations emerged as a potential protective factor, while interoceptive deficits were linked to non-suicidal self-injury and suicidal ideation. Neuroimaging pointed to the insula and prefrontal cortex as likely substrates, though this evidence is still preliminary.

My primary reason for building Pauseture was pain. The method existed while my mother suffered — and the loss of her at 53, in chronic pain, before I ever discovered Feldenkrais, is the grief underneath everything I've built.

But I've sat with a harder question since I found this research. Would she have done it? Would she have been able to?

My mother earned her PhD the same year and from the same institution as the founder of EMDR. She wrote her dissertation on drama therapy for the severely mentally ill — she understood, at a profound level, that healing happens through the body, not just the mind. She was decades ahead of mainstream psychology on bottom-up somatic approaches.

And yet. I don't think she had interoceptive awareness. I don't think she could feel herself from the inside in the way these lessons invite — in the way this research now identifies as protective. Not because she lacked intelligence or dedication. It was simply not something she practiced. She was a psychiatric nurse — working within a medical model for decades. The doctors should have the answers.  Not her.  The method that might have helped her find her way inward wasn't available to her — and even if it had been, the medical model she lived inside didn't leave room for it.

This study lands differently for me because of that. Interoceptive awareness isn't a personality trait you either have or don't. It's a skill. It can be learned — slowly, gently, with curiosity rather than effort. That's what Feldenkrais Awareness Through Movement® lessons do. And the earlier someone begins building that capacity, the more of a life it has to protect.

Why "Just Notice Your Body" Isn't the Whole Story

A large study of 456 people (cardiac tasks) and 245 people (respiratory tasks), using rigorous psychophysical testing rather than questionnaires. The researchers found that objective interoceptive performance — how accurately someone actually senses signals like their heartbeat or breath — was largely unrelated to mental health symptoms. Self-reported body-awareness did correlate with symptoms, but the researchers believe that reflects general anxious or affective tendencies, not real interoceptive skill. Their conclusion: accurate noticing itself doesn't appear to be the broad protective mechanism it's often assumed to be.

This is a big headline, and it may shape the growing belief that interoception doesn't affect mental health outcomes. But noticing your heartbeat is very different from noticing your ribs open as you breathe, or your head balancing over your pelvis in space. The study measured organ-level signal detection. Feldenkrais's version of interoception is about the body in motion — proprioception, not organ monitoring.

That distinction matters, because I think this study actually supports something I've believed for years: noticing isn't the mechanism, and Feldenkrais never claimed it was. What shifts things is what happens after the noticing — when the brain compares one way of moving to another and updates the pattern. Noticing if you can slow your breath down, or send it to your belly instead of your chest, is the entry point. It's your nervous system adopting the new pattern where the actual change in state occurs. This makes me wonder whether somatic practices built purely around noticing — without that next step of change — can create lasting change at all.

I've spent 12 years inside this practice, and this study puts words to something I've felt the whole time. So much of the wellness world treats noticing your body as the finish line — feel it, name it, sit with it, you're done. Feldenkrais was never that. The lying down, the closing your eyes, the attention inward — that's the doorway, not the destination. What happens next is the part that actually changes you: trying the movement a different way, feeling the difference, letting your brain choose the more efficient option. That's the part this research didn't measure, and it's the part I think matters most.

Ten adult ADHD patients were compared to ten matched healthy controls using standardized postural control assessments. ADHD patients showed significantly larger spontaneous sway amplitudes and velocities — meaning their bodies moved more erratically just standing still. When the platform beneath them was tilted, their body excursions were abnormally large, their response time was slower, and their long-term postural control was impaired.

The critical finding: these postural abnormalities correlated directly with the hyperactivity and impulsivity dimensions of individual ADHD symptoms. The worse the ADHD symptoms, the worse the postural control. The authors concluded that adult ADHD's major postural deficit is an impairment of stable, long-term sensorimotor behavior — a finding that maps directly onto the impulsivity and hyperactivity dimensions of the condition.

82 participants were assigned to either an upright or stooped sitting posture — crucially, through an implicit manipulation. Desk and chair heights were adjusted before participants arrived so they naturally adopted the target posture without being told to "sit up straight." This eliminated demand effects and made the findings more ecologically valid.

Results: participants in the upright posture showed measurably more positive mood (d=0.50) and significantly higher processing speed (d=0.42) compared to those in stooped postures. No differences in accuracy were found. Critically, the improved processing speed was not mediated by the mood improvement — they are independent effects. Upright posture improves cognitive speed through a mechanism beyond simply feeling better.

This study is worth naming precisely because of what makes it different from most posture research. Participants didn't know their posture was being studied. The manipulation was invisible — adjusted furniture, nothing more. And still, the upright group processed information faster and felt better.

That matters because it rules out the most common objection: that posture effects are just placebo, or people performing better because they know they're supposed to. The effect survived implicit manipulation. The body knows.

What I find most relevant for Feldenkrais Awareness Through Movement® lessons is the independence of the two effects. Better mood and faster processing speed both appeared — but they didn't cause each other. Upright posture appears to improve cognitive speed through a direct neurophysiological pathway, not just through feeling better first.

Moshe Feldenkrais understood this. He wasn't trying to make people feel better about themselves so they'd perform better. He was retraining the nervous system's relationship to gravity — the organization that underlies everything else. The researchers concluded that upright participants were engaging in more effective risk-taking, not reckless behavior. The findings suggest that ordinary environmental features — desk height, device placement, workspace design — may have small but measurable effects on emotion and decision-making quality.

This study hits home for me. I had a hunched posture for most of my life. I tried pulling my shoulders back with effort, only to be discouraged that I went right back to my patterns. I knew posture was important for how I was perceived in the world, but I didn't realize how much improving my posture would help my cognitive processing and how I felt about myself.

I began doing Awareness Through Movement® lessons for back pain, and hoped they would improve my posture. Without effort or trying, over time, I began to stand up more straight. Since we know from neuroscience that we can't erase our old patterns, when working at the computer I do find myself going back to what feels familiar — which for me is a hunch. I no longer shame myself about it. I simply think "there's my old comfy pattern," and gently tilt my pelvis forward and find the subtle upright posture — through my pelvis, not my shoulders.

Do I feel more pride? That's an interesting question. I know I feel different. In a subtle, but profound way.

A person who habitually hunches carries that posture's emotional signature everywhere they go. A systematic review and meta-analysis of 74 studies involving 4,227,297 participants across 30 countries — published in The Lancet Public Health. Researchers examined whether the association between physical activity and dementia risk varies depending on the context in which activity occurs.

The finding is striking: physical activity and dementia risk are domain-specific. Leisure-time physical activity was associated with a 24% reduced risk of all-cause dementia. Household physical activity was associated with a 15% reduced risk. By contrast, occupational physical activity was associated with a 20% increased risk of dementia, and commuting physical activity with a 10% increased risk.

More physical activity does not automatically mean better brain health. A manual worker lifting and moving for eight hours a day has a higher dementia risk than someone who doesn't. The protective benefits of movement are specific to activity that is freely chosen, novel, and mentally engaging. The authors note that occupational physical activity often involves psychosocial stress, repetitive low-complexity tasks, hazardous environments, and reduced autonomy — all of which may limit the cognitive reserve that protects against dementia.

I want to be honest about what this paper is and isn't. It's a peer-reviewed theoretical framework, not an outcomes study. The authors proposed the mechanism and called for empirical research to test it. That research is still being done.

What I can tell you is what I've observed — in my own body and in my practice.ADHD is associated with postural sway. People with ADHD tend to show larger, more erratic postural adjustments and forward head posture. These aren't cosmetic issues. Postural sway and gait quality are now well established in the literature as markers of executive function — the smoother and more efficient your movement, the better your cognitive control tends to be.What hasn't been studied yet is whether retraining movement patterns from the inside out — through the nervous system rather than through aerobic exercise — directly improves executive function in ADHD. That's the study I'd love to see.

The Clark and Mostofsky paper proposes the mechanism. The gait and cognition literature suggests the connection is real. The direct empirical study closing that loop hasn't been done yet.In the meantime, I can tell you what I've observed. Since beginning a consistent Awareness Through Movement® practice, my posture changed. My gait changed. And my ability to direct and sustain attention changed in ways that years of compensation strategies never produced. I don't think those three things are unrelated.

Reading improved neither. Effect sizes were medium to large.

I watched Moshé Feldenkrais's final training on video — recorded at Amherst in 1980–1981 — where he said with great frustration that when you stretch, you micro-tear tissue. The body then diverts its energy to repair that self-inflicted damage, meaning other healing stops happening. He called forced stretching "violence" against oneself. Willpower, he argued, overrides the nervous system's safety signals. By moving slowly and staying entirely within a pain-free zone, you teach your nervous system it is safe to lower its protective tension.

I saw this in my private practice. Clients who had been stretching for years were still in chronic pain. I would not work with them until they agreed to stop stretching. And always — their pain went away. But they didn't believe me, so they returned to stretching. And the pain returned.

A meta-analysis of 23,417 participants across six major neuroimaging datasets — including the UK Biobank, the Human Connectome Project, and the Adolescent Brain Cognitive Development study — systematically mapped the brain structural correlates of depression across the whole brain. The most robust finding confirmed what previous research suggested: reduced gray matter volume and cortical surface area in the frontal cortex, anterior cingulate, and insula.

But the most surprising finding was this: significant structural correlates of depression were also found in somatomotor regions — the brain areas responsible for sensing and controlling body movement, including the paracentral, postcentral, and precentral regions. This was unexpected. The field had focused almost entirely on higher-order cognitive and subcortical regions. Depression, it turns out, shows up measurably in the movement and body sensation areas of the brain. The authors call explicitly for increased research focus on the somatomotor system's role in depression.

And: "We cannot become conscious of a feeling before it is expressed by a motor mobilization — and therefore there is no feeling so long as there is no body attitude." He was describing, 75 years ago, the inseparability of movement and emotional state that this study now maps onto brain structure at scale across 23,417 people.

The question this study raises — and does not answer — is exactly the question Moshe was circling: does depression cause somatomotor changes, or do somatomotor changes contribute to depression vulnerability? He didn't wait for the answer. He worked with the body and watched what happened to the emotional state.

I've been waiting for a study like this.

Moshe Feldenkrais raised the question in his writing: is the depressed person hunched because they are depressed — or are they depressed because they are hunched? He saw it as a bidirectional relationship, inseparable, each reinforcing the other. What he understood intuitively, this study now begins to map empirically: depression is not only a problem of mood, cognition, or subcortical brain structures. It shows up in the somatomotor system — the brain regions responsible for sensing and organizing movement.

That changes the conversation about what depression intervention could look like.

In my training, I heard from psychotherapists who pivoted their practices from talk therapy to Functional Integration — hands-on Feldenkrais lessons. While there is value in talk therapy, you can't talk your way out of feeling better. So much lives in the body, and Feldenkrais practitioners have known this for decades.

In my own private practice, I worked with clients who carried profound depression in their bodies — in the collapsed chest, the dropped head. When they began to find the relationship between movement and ease — not through exercise, not through effort, but through slow attentive reorganization — something shifted in how they felt and thought. I was not treating depression. I was working with movement. And the emotional change followed. I even worked with licensed therapists who had hit burnout and were on leave from their private practice. One lesson and they stood up and said: "How is it possible I feel this much better?"

Bessel Van der Kolk, who called Moshe Feldenkrais "the great twentieth-century body therapist," referred a trauma patient to Feldenkrais in The Body Keeps the Score — and reported that patient's delight at his increased sense of control. The Feldenkrais community has understood the body's role in emotional healing for decades. This study begins to show why.

The study I would most love to see — and that I hope to help design someday with the right research partner — would measure somatomotor gray matter before and after a consistent Feldenkrais Awareness Through Movement® practice. If depression is associated with reduced gray matter in the movement regions of the brain, and if ATM lessons specifically and repeatedly engage those regions through novel, attentive movement, what happens to the brain over time? What happens to the depression?

This makes it one of the body's primary interoceptive organs — continuously relaying the body's internal state to the brain. When fascia becomes stiff and densified — through chronic stress, trauma, or depression — it generates what the authors call "interoceptive noise": distorted signals that degrade the brain's ability to accurately read and regulate emotional states.

Chronic stress activates the sympathetic nervous system, triggering myofibroblast differentiation that causes fascial stiffening and reduced elasticity. The study also found that after the intervention, none of the participants mentioned ignoring or distracting from discomfort as a coping strategy — instead, all 14 participants reported using at least one somatic self-regulation strategy including breath, movement, and attentional awareness.

For most of my life I had forward head carriage and a hunched posture. At 47, I genuinely believed I was stuck with it. The pattern felt fixed — structural, permanent, just who I was physically. But over time, without forcing anything, my posture began to change. My forward head carriage improved. My spine found more mobility.

What strikes me is what it says about the style of movement, not the intensity. We've been told for decades that exercise helps depression. Walk more. Run more. Move more. And that's true as far as it goes. But this study says something more specific and more interesting: it's not just whether you move, it's how you move. A depressed walking style — slower, more downward, less arm swing, more collapsed posture — produces a depressed memory bias, independent of how fast you're going.

Moshe Feldenkrais understood this intuitively. His lessons don't ask you to exercise more. They ask you to move differently — with more awareness, more efficiency, more ease. They change the quality and organization of how you move. My primary reason for building Pauseture was pain. The method existed while my mother suffered — and the loss of her at 53, in chronic pain, before I ever discovered Feldenkrais, is the grief underneath everything I've built.

But I've sat with a harder question since I found this research. Would she have done it? Would she have been able to?

My mother earned her PhD the same year and from the same institution as the founder of EMDR. She wrote her dissertation on drama therapy for the severely mentally ill — she understood, at a profound level, that healing happens through the body, not just the mind. She was decades ahead of mainstream psychology on bottom-up somatic approaches.

And yet. I don't think she had interoceptive awareness. I don't think she could feel herself from the inside in the way these lessons require — in the way this research now identifies as protective. Not because she lacked intelligence or dedication. But because that capacity, for many people, was never developed. Nobody taught her to sense herself. She was a psychiatric nurse — working within a medical model for decades. The doctors should have the answers.  Not her.  The method that might have helped her find her way inward wasn't available to her — and even if it had been, the medical model she lived inside didn't leave room for it.

This study lands differently for me because of that. Interoceptive awareness isn't a personality trait you either have or don't. It's a skill.
